Helpful notes

  1. Please note that completion of this booking form does not constitute a booking for the LGA General Assembly on 26 June 2012, which is open to LGA member organisations only. Booking forms and information in relation to the General Assembly will be sent under a separate cover.
  2. Name of organisation and delegate details: please complete these sections as you would intend them to be reproduced on badges, delegate list, etc. We will endeavour to meet these requirements subject to space availability on our database fields.
  3. Unless otherwise specified (on a separate sheet) all joining instructions will be sent by to the email address provided on the booking form.
  4. Contact name, email address, telephone and fax numbers: please provide  individual and central booking contact details.
  5. Political party: this information has been requested by the LGA political groups, please complete only if you are a local authority councillor.
  6. Accommodation: please note that all accommodation must be booked separately online by logging on to www.lggroupinbirmingham2012.com
  7. Please notify us of any special dietary, BSL Interpreter, religious or mobility arrangements you require by 17 May 2012. As some of these services are provided through external suppliers and due to the pressures on them we cannot guarantee your requirements will be met if not indicated by this deadline.
